Our home and healthcare services commence once we are notified through the assessment of your Doctor and are authorized by your family members or insurance company to care for you in the comfort of your home.
Jarme Registered Nurses (RN) and Care Managers will schedule a visit to assess your needs, based on your Physician's recommendations. We will work with your Physician and family to create an individualized plan of care that suites your needs and provides care that is convenient for you. We then identify the right professionals in our team whose knowledge and experience best suite your needs.
We will also reassess your needs continuously while communicating with your Physician to change your plan of care based on recommendations from our professional team members and the approval of your Physician.
